Myndset youth program

Myndset is a free service for young people aged 12 to 25 who are beginning to experience moderate to severe mental health concerns.

Our aim is to offer care coordination, which could include counselling and creating support networks, in order to improve wellbeing. Our program is flexible and tailored to individual needs and goals.

Myndset is a mental health team of social workers and clinical psychologists.

We collaborate with clients in areas such as:

  • Mental health and wellbeing
  • Family supports
  • Relationships and social interactions
  • Basic life skills and everyday tasks
  • Engagement in school or work

Here's what happens

The first few appointments will be focused on understanding the needs of the young person. This will involve identifying the young person’s goals and mental health concerns and history. Clinicians will ask about domains such as social relationships, school and work, interests, strengths, substance use, and risk.

Clinicians use evidence-based screening questionnaires to assess needs and monitor changes over time.

Clinicians draw on evidence-based approaches to respond to the young person’s needs.

Young people may receive counselling, referrals to additional appropriate supports, liaison with schools, and/or outreach. The young person’s GP will be regularly updated on their program journey.

Eligibility

Myndset provides a no cost service to young people living on the Sunshine Coast or in Gympie who meet these criteria:

Referral to the program

Referrals to the Myndset are via Head to Health, headspace centres and Queensland Hospital and Health Services.

GPs can refer patients by completing the PHN referral form for ‘PHN Mental Health Intake Stepped Care Services’ and send it to Head to Health via fax or Medical Objects.
